We strongly encourage you to use the secure patient portal. This will save you time filling out necessary forms in the office and allow us to enter the forms directly into your chart. Please be sure to completely fill out all 4 sections (My Information, Allergy/Health History, Financial Agreement, Privacy Practices) prior to your appointment. If asthma is a concern, also fill out the Asthma Control Test. Please note it sometimes takes 24 hours for the portal to be active and allow you to access the appropriate forms.
In case you want to print and save extra copies of our office's financial agreement and Privacy Practices: You can also print/fill out/bring new patient forms for your first visit, but please consider using the patient portal above: If you would like your medical records sent to us from another doctor, please fill out the records release form below:
